Private Membership Association (PMA) Membership & Disclosure Agreement

This agreement must be signed prior to any discussion, purchase, or participation in PMA wellness activities, including access to LifeWave patch training or educational resources.

1. Welcome to Stargate Clinic PMA

Stargate Clinic operates as a Private Membership Association (“PMA”) offering access to wellness products, education, and non-medical services exclusively for members.
Membership is required before you may purchase wellness products (such as LifeWave IceWave patches) or access our training resources.

2. Membership Fee and Benefits

• $25 Activation Fee – Deferred:
  I understand there is a one-time $25 membership activation fee. This fee will not be collected until six (6) months after signing this agreement, provided I remain an active member at that time. During the first six months, all PMA rights and responsibilities apply.

• Included Benefits:
  My membership entitles me to:

  • One (1) foot wellness consultation (non-medical, wellness education only)

  • One (1) foot Podiatry consultation (any necessary procedures will be billed insurance or concierge)

  • Ongoing patch training and wellness visits as needed (up to 10 minutes per session)

  • Access to member-only education and product resources

  • Communication and updates from Stargate Clinic PMA

3. Separation of PMA Wellness Services and Podiatric Medical Care

I acknowledge and understand:

• PMA Services:
  All patch training, education, and wellness consultations provided through Stargate Clinic PMA are non-medical and do not include diagnosis, evaluation, or treatment of foot, ankle, or other medical conditions.

• Medical Office Services:
  Any Podiatric medical care (diagnosis, treatment, procedures, or medical advice) will be provided separately through the medical office (Advanced Foot Care Center or similar).

• Insurance and Billing:

  • All podiatric medical services are billed separately through insurance or direct payment and are never included in PMA membership.

  • No services provided through the PMA will ever be billed to insurance.

• No Overlap:
  PMA membership does not entitle members to free, discounted, or priority medical care. Membership is strictly for wellness, education, and product access.

4. Disclosures & Member Acknowledgements

By signing below, I acknowledge and agree:

• I am joining Stargate Clinic PMA of my own free will for private education, wellness consultation, and access to non-medicated wellness products.

• I understand that LifeWave IceWave patches and other products provided are not drugs, are not FDA-approved to diagnose, treat, cure, or prevent any disease or condition, and are for wellness purposes only.

• The information, products, and training provided by Stargate Clinic PMA are not a substitute for medical advice or care from a licensed healthcare provider.

• Results may vary; there are no guarantees or warranties regarding wellness outcomes.

• My participation and/or purchase is entirely voluntary, and I have not been pressured or promised specific results.

• I can withdraw my membership or opt out before the $25 fee is charged, with no penalty.

5. Privacy & Confidentiality

All discussions, purchases, and training within the PMA are private and intended solely for members.
Personal information will be kept confidential and not shared outside the PMA except as required by law.

6. Consent and Signature

By signing below, I agree to all the above terms and consent to participate in the Stargate Clinic PMA, understanding that:

• The $25 fee will not be charged until six months after enrollment (with advance notice)

• All wellness visits under PMA are separate from podiatric medical care

• I may access patch training and educational resources upon signing

• I have read and understand the separation between PMA and medical office/insurance billing
